The Foster Parent Application is a comprehensive document used to gather detailed information about you, your household, background, caregiving experience, and overall readiness to become a foster parent. Because this application is thorough and requires thoughtful, accurate responses, please plan for additional time to complete it in full. We recommend completing this form in a quiet setting where you can carefully review each section.

Next Steps Guide
The Next Steps Guide outlines the upcoming steps in your foster parent application and onboarding process with CBHS, helping you understand what to expect as you move forward. This document serves as a helpful roadmap for completing required paperwork, training, interviews, and licensing requirements. The online version is provided as a backup resource in the event your mailed or previously provided copy is misplaced, ensuring you always have access to important process information and instructions.

Responsible Individuals List (RIL)
The Responsible Individuals List (RIL) form is a required background screening document used as part of the foster parent application and licensing process. This screening helps ensure child safety and supports compliance with state foster care licensing requirements. This form should be completed accurately and typed directly into the document whenever possible. If electronic completion is not possible, please download the fillable PDF, complete the form in full, and provide the required signatures before submission.

Reference Forms
Reference forms are required as part of the foster parent application and licensing process to help CBHS gain a better understanding of each applicant’s character, reliability, and suitability to provide care for children in foster care. Each prospective foster parent must have three separate reference forms completed on their behalf. For example, if two individuals are applying together as a married couple or joint household applicants, Applicant 1 must provide three references and Applicant 2 must also provide three separate references, for a total of six completed reference forms.
